All signers must provide valid photo ID. 

Acceptable forms of ID:

US Passport

US state issued ID 

US state issued  Drivers License

US Military ID

Mexico or Canada Drivers License

Foreign Passport with US INS stamp 

If an  ID is not available for one or all signers two witnesses must be used, the witnesses must be able to verify the signers and provide one of the listed ID's each.
